30-Day Notice of PERMANENT Practice Closure – EFFECTIVE DECEMBER 24TH 2025
We are writing to let you know that High Horizon Medical P.C. will permanently close on December 24th, 2025. Until that date, our focus is helping you transition care safely and access your medical records.
YOUR OPTIONS FOR ONGOING CARE:
Choose a new primary care provider (PCP): We can provide in-network referrals based on your insurance.
Email WHPC@WHPC396.COM with your full name, date of birth, and insurance plan, and we will share nearby options accepting new patients.
Urgent needs: For medical emergencies, call 911 or go to the nearest emergency department. For urgent but non-emergency issues, consider urgent care or your new PCP.
ACCESSING YOUR MEDICAL RECORDS (PHL §18 & HIPAA):
You have a right to inspect and receive copies of your records.
How to request: Email MEDICALRECORDS@WHPC396.COM (or mail to our address above) with:
- Full name and date of birth
- Best phone number
- What you need (entire chart or specific dates/items)
- Where to send (your email/mailing address or your new provider’s secure fax/email)
- Preferred format (patient portal, encrypted email/ptent with New York Public Hassword-protected PDF, regular email*, or paper by mail)
*If you choose regular (unencrypted) email, please reply “I ACCEPT” after we provide a brief risk notice.
Timing: We will offer inspection within 10 days of your written request and provide copies within a reasonable time thereafter, consistent with New York Public Health Law §18 and HIPAA.
Fees (New York rules):
- Paper copies: up to $0.75 per page.
- No charge for records requested for government benefits/appeals.
- We will notify you of any fees before sending copies.
Records Custodian: High Horizon Medical P.C., Compliance Department.
The records custodian will remain available after December 24th, 2025, at the email listed above.
Retention (NY): adult records ≥ 6 years from last entry; minors/obstetrical: 6 years and until 1 year after the patient turns 21.
PRESCRIPTIONS + REFERRALS DURING WIND-DOWN:
We cannot schedule routine office visits after the closure date. For continuity, we may provide short, clinically appropriate bridge refills for non-controlled maintenance medications on a case-by-case basis before December 24th, 2025. For ongoing medication management and new treatment plans, please establish care with your new PCP. We can send your records directly to your new clinician upon your written direction (HIPAA authorization).
